What Testosterone Level Is Actually Considered Low?
If you've ever had your testosterone checked, there's a good chance you looked at the result and immediately asked:
"Is that low?"
It sounds like a simple question. You might expect there to be one number separating "normal testosterone" from "low testosterone."
In reality, diagnosing low testosterone is more nuanced.
A testosterone level is important, but the number is only one part of the picture. Symptoms, the timing of the blood test, repeat measurements, free testosterone, SHBG, medications, medical conditions, and the underlying cause can all matter.
So, is a testosterone level of 350 low? What about 300? Or 250?
Let's break it down.
What Is Considered a Low Testosterone Level?
A commonly used clinical threshold for low total testosterone is approximately 300 ng/dL.
The Endocrine Society has recently described a common clinical threshold near 300 ng/dL and emphasizes that diagnosis should generally involve at least two early-morning testosterone measurements along with compatible symptoms.
However, 300 isn't a magic number.
Different laboratories use different reference ranges and testing methods. One large harmonization study using standardized testing established a lower reference limit of approximately 264 ng/dL in healthy, non-obese young men.
This is one reason testosterone shouldn't be interpreted by simply asking whether your result has an "L" next to it.
Why Symptoms Matter
Low testosterone — or male hypogonadism — isn't diagnosed based on a laboratory number alone.
Clinical guidelines recommend diagnosing hypogonadism when a man has symptoms or signs consistent with testosterone deficiency together with consistently low testosterone concentrations.
Symptoms can include:
Decreased sex drive
Fewer spontaneous or morning erections
Erectile dysfunction
Reduced energy
Decreased muscle mass
Fertility problems
Changes in mood or concentration
Reduced bone density
Hot flashes in cases of severe testosterone deficiency
But here's where things get complicated.
Fatigue doesn't automatically mean low testosterone.
Neither does weight gain, poor motivation, decreased gym performance, or erectile dysfunction.
Sleep disorders, obesity, depression, thyroid disease, medications, chronic illness, poor nutrition, and many other conditions can cause similar symptoms.
That's why treating a testosterone number without evaluating the person attached to it isn't a good approach.
Why Testosterone Should Usually Be Checked in the Morning
Testosterone isn't static throughout the day.
Levels fluctuate and are generally highest in the morning, particularly in younger men. Testosterone can also vary significantly from one day to another.
For this reason, testosterone testing is generally performed early in the morning, and an unexpectedly low result should usually be confirmed with another measurement.
The Endocrine Society recommends confirming the diagnosis with a repeat morning fasting total testosterone measurement.
Its patient guidance describes obtaining at least two early-morning tests, typically between approximately 6:00 and 10:00 AM.
So one testosterone level drawn at 4 PM shouldn't necessarily determine whether someone has hypogonadism.
Total Testosterone Isn't the Entire Story
This is one of the most important concepts for patients to understand.
Most people are familiar with total testosterone, which represents the testosterone circulating in the bloodstream in several forms.
But only a small portion of testosterone circulates freely.
Much of it is bound to proteins, particularly:
Sex hormone-binding globulin (SHBG)
Albumin
The unbound portion is referred to as free testosterone.
This matters because two men could have identical total testosterone levels but different amounts of testosterone available to tissues.
What Is SHBG?
Sex hormone-binding globulin, or SHBG, is a protein that binds testosterone and other sex hormones in the bloodstream.
Changes in SHBG can alter the relationship between total and free testosterone.
For example, someone with relatively high SHBG may have an acceptable-looking total testosterone level while having a lower free testosterone level.
Conversely, someone with low SHBG may have a lower total testosterone measurement while their free testosterone tells a somewhat different story.
This is why the Endocrine Society recommends using accurate measurements of total testosterone and free testosterone when clinically indicated, rather than applying one laboratory number to every patient.
Is a Testosterone Level of 350 Low?
This is exactly the type of result where context becomes important.
A total testosterone of 350 ng/dL isn't automatically diagnostic of testosterone deficiency.
You would want to know:
Was the test performed early in the morning?
Was it repeated?
Does the patient have symptoms?
What is the free testosterone?
What is the SHBG?
Are medications affecting testosterone?
Is obesity or another medical condition contributing?
Are there sleep problems or possible sleep apnea?
Is there evidence of a testicular or pituitary problem?
The appropriate question isn't simply:
"Is 350 low?"
It's:
"What does 350 mean in this particular patient?"
What About a Testosterone Level Below 300?
A consistently low morning total testosterone below approximately 300 ng/dL along with compatible symptoms provides stronger evidence for testosterone deficiency.
But even then, the evaluation shouldn't necessarily stop.
The next question should be:
Why is the testosterone low?
Finding the Cause of Low Testosterone
Testosterone deficiency can result from problems involving the testes themselves or from the hormonal signaling system that tells the testes to produce testosterone.
Two hormones that can help distinguish between these possibilities are:
Luteinizing hormone (LH) and follicle-stimulating hormone (FSH).
Clinical guidelines recommend using LH and FSH to help differentiate primary hypogonadism, in which the problem originates primarily in the testes, from secondary hypogonadism, involving the pituitary or hypothalamic signaling system.
Depending on the situation, additional evaluation might include prolactin or other testing.
This matters because not every man with low testosterone should automatically receive the exact same treatment.
Can Lifestyle Affect Testosterone Levels?
Absolutely.
Body weight, metabolic health, sleep, nutrition, illness, medications, and other factors can influence testosterone.
Obesity in particular is frequently associated with lower testosterone concentrations.
In its July 2026 statement, the Endocrine Society emphasized evaluating potentially reversible contributors before starting testosterone treatment. For appropriately diagnosed men whose hypogonadism is associated with overweight or obesity without another identified cause, the Society describes weight loss as typically being first-line therapy.
That doesn't mean every case of low testosterone can be fixed through lifestyle changes.
It means the underlying cause matters.
Does Low Testosterone Automatically Mean You Need TRT?
No.
Testosterone replacement therapy can be an appropriate treatment for men with properly diagnosed hypogonadism, but a low laboratory result by itself doesn't automatically mean someone should begin testosterone.
The decision should consider:
Symptoms
Repeat laboratory testing
The underlying cause
Fertility goals
Medical history
Potential benefits
Potential risks
Appropriate monitoring
Fertility is particularly important to discuss because exogenous testosterone can suppress sperm production. The Endocrine Society recommends against starting testosterone therapy in men planning fertility in the near term.
What Should Be Checked Before Starting TRT?
The exact evaluation should be individualized, but a proper assessment may include more than simply total testosterone.
Depending on the patient's age, symptoms, history, and clinical situation, evaluation may involve:
Total testosterone
Repeat morning testosterone
Free testosterone when indicated
SHBG
LH and FSH
Complete blood count/hematocrit
PSA when appropriate
Prolactin when indicated
Evaluation for contributing medical conditions
Discussion of fertility goals
The purpose isn't simply to determine whether testosterone can be prescribed.
It's to determine why someone feels the way they do and whether testosterone therapy is actually an appropriate treatment.

At Jersey Shore TRT, we don't believe testosterone care should revolve around chasing a single number.
A testosterone result needs context.
A man with a total testosterone of 290 ng/dL and no symptoms isn't necessarily the same as a man with a level of 290 ng/dL who has experienced a significant decline in libido, spontaneous erections, and other compatible symptoms.
Likewise, someone experiencing fatigue and poor motivation shouldn't automatically be told testosterone is the answer simply because his level is toward the lower end of a laboratory reference range.
Good hormone care requires looking at symptoms, laboratory results, medical history, potential underlying causes, fertility goals, and the individual patient.
The goal shouldn't be to achieve the highest testosterone number possible.
The goal should be to correctly identify testosterone deficiency when it exists and develop an appropriate, medically supervised treatment plan.
